The Osburn AC01298 is a 5" fresh air intake kit that brings outside combustion air to an Osburn wood insert. An adapter fastens to the left or right end of the insert and connects to an insulated duct that runs to an outside wall termination. Osburn lists it for its 2000-I, 1700-I, 3500-I, Inspire 2000-I, Matrix, Matrix 1900 and Matrix 2700 inserts.

How does it install?
The 3500-I manual has you remove the small plate on the end of the insert, fasten the intake adapter with four screws, and clamp on an insulated flexible duct that runs to a termination on the outside wall.
What else do I need?
A 5" insulated HVAC duct rated ULC S110 and/or UL 181, class 0 or 1, such as the AC02090, and an outside wall termination.
Do I need to check it?
Yes. The 2000-I manual notes outdoor air ducts can be affected by wind and should be checked for soot at least once a year.
