Do I need a different screen for each size?

No. Napoleon lists one screen, STS4758, covering both the 4/7 and the 5/8 Silhouette terminal.

Is a stand-off needed too?

Napoleon says a stand-off may be required for brick and vinyl applications. Stand-offs are listed separately, for example the SOST58B for 5/8 brick.
